Community Rx | Liability Waiver

The Community Rx Program (the “Program”) is not designed to treat or cure any individual medical condition. PlantPure, Inc., as well as its past or present officers, directors, employees, and agents, do not engage directly or indirectly in diagnosing, dispensing medical advice, endorsing, recommending, or prescribing the use of any medical product, therapy or service as a treatment for disease or sickness whether physical or mental.

Information provided by the Program is not intended to be medical advice and should not be relied on as such. PlantPure, Inc. offers general nutrition information in the Program for educational purposes. The information provided through the Program is not intended to substitute for consultation with the participant’s physician or healthcare provider, nor is it specific medical advice regarding a participant’s individual health concerns.

Acknowledgment and Assumption of Risk

I understand that this Program involves a pronounced change in my diet. I understand that these changes may cause rapid healing, which could affect my need for medication(s). Importantly, if I am taking any medication(s) for the treatment of blood pressure and/or diabetes, I understand that I should consult with a physician before, during and after participation in the Program so that he/she can monitor and adjust my medication(s) if necessary. And even if I am not taking one or more of these medications, I understand that I should still inform my physician of my intended participation in the Program.

I also understand that there are potential risks of which I may not presently be aware. I voluntarily elect to participate in this Program, and I hereby agree to accept and assume any risks resulting from my participation.

Waiver of Liability and Indemnification

In consideration for being allowed to voluntarily participate in the Program, on behalf of myself, my personal representatives, heirs, next of kin, successors and assigns, I forever waive, release, and discharge PlantPure, Inc. and its agencies, officers, and employees from any and all negligence and liability for personal injury or claims of any nature which may hereafter accrue to me and/or my estate as a direct or indirect result of my participation in the above referenced activity or event; and defend, indemnify, and hold harmless PlantPure, Inc., its agencies, officers and employees from and against any and all claims of any nature, including all costs, expenses and attorney fees, which in any manner result from my actions during my participation in the Program.

This release, indemnification, and waiver shall be construed broadly to provide a release, indemnification, and waiver to the maximum extent permissible under applicable law. I, the undersigned participant, affirm that I am at least 18 years of age and am freely signing this agreement. I have read this form and fully understand that by signing this form I am giving up legal rights and/or remedies that may otherwise be available to me regarding any losses I may sustain from my participation in the Program. I agree that if any portion of this Agreement is held invalid, the remainder will continue in full legal force and effect.
